Where can I get an Evaluation for court?
Where can I go to complete my DUI education / Treatment?
Northern Illinois covers several counties: McHenry, Kane, Cook, DuPage, Kendall, Will, Grundy and Kankakee; each with different regulations for getting a DUI Evaluation and treatment. Cook and DuPage counties require that you get an evaluation for a DUI charge in that county, but you are free to go to DUI classes at any DASA/ SUPR licensed site in the state of Illinois. Kane county has an “court approved” list of DUI providers where you can get an evaluation or complete recommended treatment, but this is NOT a requirement, and you are free to seek out any licensed agency you choose. All DASA / SUPR licensed agencies can be found on the states website below.

Also, use the menu below to find the closest provider to you by address or zip code, or see the map below to help find the provider closest to you. Always call to make an appointment, ask for available hours and prices. These will all vary significantly between provider.
All licensed DUI providers are able to provide an evaluation for the Secretary of State (It’s the same form). SOS evaluations are often more complicated due to various reasons, and therefore often priced higher. You should be aware that your evaluation for the SOS is only good for six (6) months from the date of completion, so be aware of agency costs for updated evaluations. You may only have this update done at the agency where you got your original evaluation done OR the agency where you did your treatment hours.
Dealing with a revocation is often done after court proceedings have concluded, and is a CIVIL matter, not a CRIMINAL one. Therefore, you are at liberty to seek council or legal advice, but this is not required. DUI providers can help you to access all the information you may need prior to a hearing , either formal or informal.
You will need to provide the following information for a DUI evaluation:
SSS
Driving Abstract.
